Our doctors and staff are dedicated to providing excellent dental care to our patients. In an effort to encourage
good dental health, we developed a unique “in-house” dental program for patients who cannot obtain any other type of
coverage.
The “Purely Dental” Plan is an annual reduced-fee savings plan for families and/or individuals who do not have any
other type of coverage. With the “Purely Dental” Plan you can receive the quality dental care you need and deserve
at an affordable rate.

Purely Dental Plan Rates
Rates are dependent on periodontal condition when the patient presents for initial visit. The patient can fall into the routine plan or the periodontally enhanced plan. Rates are discussed in-office once the initial visit is completed
Purely Dental Plan Includes:

Routine Dental Exams (2 per year)

Bitewing X-Rays (1 per year)

*Adult or *Child Cleaning (2 per year) *(excluded from perio plan)

Oral Cancer Screening (1 per year)

Periapical X-Rays (2 per year)

Panorex (1 every 5 years)

Fluoride (2 per year : 12 yrs old or younger)

Discounted In-office laser whitening

15% Off Additional Preventative Services (sealants)

20% Off Basic Services (fillings, extractions) & Scaling & Root Planing

25 % Off Major Services (crowns, bridge, denture, partial, onlay/inlay), Endodontics (root canal) & Implants.
Purely Perio Plan Includes:
All of the same offered services, discounts and specials as the Purely Dental Plan with the exception of/and
including:

Perio Maintenance Cleaning (4 per year)

Fees and services guaranteed for 1 year to date

Plan fees are NON_-REFUNDABLE for any reason

Qualified child is 12 years old or younger

Services are for covered member only and cannot be transferred to another

This is NOT dental insurance and cannot be combined with insurance coverage

Services cannot be rolled over into a new year.

$150.00 FAIL FEE FOR ALL NO SHOW, CANCELED, CHANGED OR FAILED APPOINTMENTS WITHOUT A MINIMUM OF 48 HOURS
